67 year old asymptomatic man with liver mass detected on health screening. Serum alpha-fetoprotein was normal (this is noted in about half of hepatocellular carcinomas).
The right lobe of the non-cirrhotic liver shows a solitary, nonencapsulated, tan, fleshy mass with areas of haemorrhage. There is tumour invasion into the right portal vein.
