PEACE - Preparation for End-Of-Life with Advance Care Education

Project Description:

At the end-of-life, peaceful care is dependent on clear unity of values among the family members and the patient. The goal of this project is to prepare caregivers for EOL decision making with patients and clinicians by facilitating values discussions among caregivers and patients.

Research shows that the challenges to this conversation are inaccurate substituted judgement by caregivers, difficulty applying values to treatment decisions, and that attitudes towards life extension and quality of life may shift with age and life-limiting disease. Increasingly, thought leaders in the field of ACP have recognized a need to shift from planning and documentation to preparing caregivers to be informed advocates for the care recipient.

These are important initiatives to bring to Singapore. However, various local culture and standards practice. We want to create better end-of-life care conversations that lead to more engagement in advance care planning decisions, reduce decisional conflict among caregiver, and improve bereavement adjustment among caregivers/surrogates.
