The Senior Eye Rehabilitation (Seer) Programme takes an integrated and comprehensive approach to care that addresses the goals of the ‘whole person’ with low vision. Such goals may include regaining independence, managing the home, taking care of personal needs, and participating in the community, work and leisure activities.
Through a team-based multidisciplinary approach involving ophthalmologists, optometrists, occupational therapists, care coordinators and community-based services, the programme aims to help seniors with low vision age-in-place in their homes and within the community through a customised visual rehabilitation plan.
