Presented by Dr Dion Candelaria, from The University of Sydney, and hosted by NUS Nursing’s A/Prof Wilson Tam, the seminar will delve into how cardiovascular rehabilitation is reframed from short-term, post event care to a sustained, lifelong model that supports people across the full trajectory of heart health.

This seminar will also examine how stronger system coordination and integrated pathways can improve patient outcomes and experiences, and support long‑term engagement. It outlines opportunities to modernise delivery embed prevention and person-centred care into routine practice. Reframing cardiovascular rehabilitation as a lifelong health strategy and leveraging international advocacy to drive equitable access and implementation, strengthens systems and supports sustained cardiovascular wellbeing
