Managing diabetes with NUH pilot programme

Chronic conditions such as diabetes have become increasingly prevalent primarily due to medical advancements and lifestyle changes over the years. Dr Yew Tong Wei, Assistant Professor at Department of Medicine in NUS Medicine, and Consultant at Division of Endocrinology at the National University Hospital (NUH), explained that it is important for patients to take charge of their conditions, as patients only spend approximately four hours a year with healthcare professionals, whereas nearly 6000 hours per year are spent managing their conditions on their own. Decisions made by patients outside of medical supervision such as daily habits may have effects on their health conditions.
An NUH pilot programme titled “Year of Care” was launched in September 2017 to help diabetic patients. Under the programme, patients undergo a health check two weeks prior to their annual consultation. This includes height and weight measurements, blood and urine tests, and eye checks. Doctors will then advise patients based on their medical reports on the precautions to manage their chronic conditions and improve their quality of health.
