The increasing number of community-dwelling older adults has led to a growing demand for community support, services and facilities. New knowledge is needed to improve existing guidelines and provide new interventions for continuity of care.
In this theme, research drawing on trends and lived experiences of older adults will be conducted and evaluated to develop new insights towards maintaining optimal health and function of community-dwelling older adults to prevent or delay admissions to hospitals.
