Cardiovascular diseases continue to be a major cause of morbidity and premature mortality, and account for a disproportionate percentage of healthcare costs.
There is a need for healthcare service delivery platforms that simultaneously reduce healthcare burden and increase effective self-management at home and in the community. By leveraging on technological innovation, the Chronic Illness and Long-term Care research in NUS Nursing endeavors to spearhead the development of technology-enhanced secondary prevention delivery platforms that revolutionise cardiovascular disease management – in a way that is accessible, flexible, and sensitive to the needs of patients and their caregivers.
