Chronic diseases are long-term medical conditions that are generally progressive. Statistics show that Singapore’s ageing population and adoption of unhealthy lifestyles have contributed to the increasing prevalence of chronic diseases.
This course is designed for healthcare professionals caring for patients with chronic diseases in the community setting.
This programme allows participants to:
5 full day from 9.00 a.m. to 5.00 p.m.
Any healthcare professional involved in care delivery of clients with chronic diseases.
2025: 17/2, 19/2, 20/2, 24/2 and 26/2
Learners must record at least 75% attendance, and pass all assessment components, to be eligible for SSG funding.
