There is one intake per academic year, in August. Application for the August intake opens from 1 April to 15 May every year.

Programme Overview

The Graduate Certificate in Integrated Health is designed to equip healthcare professionals with the knowledge and skills to provide integrated, holistic, patient-centred care across various healthcare settings. Care, covering acute and chronic disease management, mental health, ageing, and technology in healthcare from an integrated care perspective. This programme is relevant to healthcare professionals seeking to develop leadership skills in healthcare organisations or in transition into roles focused on care coordination, population health management, or quality improvement in patient outcomes and healthcare delivery efficiency. 

This programme is offered on a part-time work-study mode. The minimum and normal candidature period is over 2 regular semesters, 1 year and the maximum candidature is 2 years.

Aims of Programme

The programme aims to deepen competencies in the integration of healthcare services and practices to improve patient outcomes and healthcare delivery efficiency, adopting a work-study approach. 

Upon completion of the programme, graduates will be able to:

  1. Demonstrate a deep understanding of integrated care principles and practices.
  2. Develop patient-centred care coordination plans and manage transitions across settings.
  3. Apply evidence-based integrated care practices in managing acute and chronic conditions in different healthcare settings.
  4. Deliver effective education programmes to engage and empower patients and families.
  5. Apply interdisciplinary teamwork and communication skills to support integrated care
  6. Analyse population health data and design preventive care programmes.
  7. Implement value-based care and address the unique needs of special populations

Programme Structure

The 12-unit part time programme comprises 3 courses, to be offered over 1 year (2 regular semesters). The structure allows for flexibility and recognition of learning at each stage:

  1. On successful completion of all 3 courses with a minimum GPA of 2.00 (cumulative across all courses), students will be awarded the Graduate Certificate in Integrated Health.
  2. Partial Completion (Exit Before All 3 Courses Completed):
    • If a student exits the programme before completing all 3 courses, they will be awarded a Certificate of Competency for each course they have successfully completed, provided they achieve a minimum GPA of 2.00 for that course.
  3. Stacking Towards Certification:
    • Each course completed with the required GPA contributes toward the overall Graduate Certificate.
    • Students who initially exit early but later return to complete the remaining courses (within the specified maximum candidature period) can stack previously earned Certificates of Competency towards the Graduate Certificate, as long as the cumulative GPA requirement is met.

Applicants are required to register for each of the modular courses singularly.

There are two consecutive mini semesters in each regular semester. The first mini-semester (Mini-semester A) comprises 6 weeks of instruction, 1-week break (Reading Week) and 1 week of examinations. 

The second mini-semester (Mini-Semester B) comprises 6 weeks of instruction, 1-week break (Reading Week) and 2 weeks of examinations (which is the same period as the end-of-semester examinations).
