The Patient Safety Briefing session, regularly conducted in NUH, aims at learning from incidents, improving systems and promoting just culture. This session will highlight medication errors, falls and other patient safety incidents with the objective of learning from these errors. A glimpse of the e-learning module on Medication Safety will also be shared at the session. Learning from error is one of the best ways to prevent repeat occurrences. In a culture of safety, all errors should be viewed as learning opportunities, and information gathered from error analyses should be used to improve medication safety processes.
