Transforming the delivery of health care to maximise value and improving clinical outcomes while simultaneously reducing costs, is fundamental to reforming health care. Achieving this goal requires fundamental (too many fundamental) changes to healthcare delivery that better align people, processes, and technology. As such efforts gain momentum, they increasingly demonstrate the importance of weaving continuous and systematic evidence-generating medicine activities into routine clinical practice. This presentation will highlight the development and implementation of a pilot programme incorporating key features of such a “learning health system”, as well as its resulting improvements in care and reductions in costs.
