Alert Worsening conditions And Report Early (AWARE) is a patient education programme developed for hospitalised patients to recognise and report symptoms of acute deteriorating condition.

The programme consists of a 30-min one-on-one interactive instructional material. It addresses three areas:

  1. be alert;
  2. recognise worsening conditions and;
  3. report immediately when experiencing early symptoms.

A cluster randomised controlled trial supported the effectiveness of AWARE programme in enhancing the self-efficacy of hospitalised patient in recognising and reporting acute deteriorating conditions.23
