Card Access

If you are not issued with a staff card or coming into the department as a visitor, a temporary card can be applied online.
The NUS Medicine Card Access Application System has been launched and can be accessed via this link and user guide.
Request Safety lead or PI to apply on your behalf after completion of (1) Safety Induction Online Quiz (passed with >80%) and (2) fill in and complete the Safety and Health Orientation Form.

Exit Declaration

All personnel exiting the department, are to complete the exit declaration form and submit it to Bchsafety (mailbox 22 next to reception counter) before returning your Staff Card / Temp Card / i-MediCare Card / NUS 16GB secured flash drive to General Office.
