1a.\u00a0<\/strong>To investigate novel interventions for vascular cognitive impairment.<\/strong> <\/span><\/h2>\n

The WHO Guidelines on risk\u00a0<\/span>reduction of cognitive decline and dementia (<\/span>Risk reduction of cognitive decline and dementia: WHO guidelines)\u00a0<\/span><\/a>provide evidence-<\/span>based recommendations on lifestyle behaviours and interventions to delay or prevent cognitive decline and\u00a0<\/span>dementia. However, how best to implement such interventions and importantly whether a multi domain\u00a0<\/span>lifestyle intervention is effective, safe and globally feasible remain important research topics.\u00a0Hence\u00a0we\u00a0<\/span>propose to conduct a large community-based innovative trial as part of the\u00a0World Wide\u00a0FINGERS\u00a0<\/span>interdisciplinary network\u00a0<\/span>toshare<\/span>\u00a0experiences, harmonize data and plan joint international initiatives for the\u00a0<\/span>prevention of cognitive impairment or dementia (<\/span>https:\/\/www.alz.org\/wwfingers\/overview.asp<\/span><\/a>). The trial will\u00a0<\/span>target subjects with high risk to develop cognitive impairment and dementia. The main goal is to determine\u00a0<\/span>the efficacy and safety of multimodal lifestyle interventions together with intensive BP lowering. We\u00a0<\/span>hypothesize that these interventions have the potential to reduce cognitive decline and severity of\u00a0<\/span>CeVD<\/span>\u00a0<\/span>and other VCI biomarkers which is closely integrated with\u00a0<\/span>Themes 2.<\/span><\/b><\/p>\n

Progress:<\/strong><\/span> We have finished recruitment for the SINGER study, and currently doing follow up visits for all the recruited participants.<\/strong><\/span><\/p>\n

1b.\u00a0Health Services Research in VCI. (Linked to Theme 3)<\/strong><\/h2>\n

We shall examine the results from the <\/span>interventional trial in\u00a0<\/span>Theme 1\u00a0<\/span><\/b>and the biomarker studies in\u00a0<\/span>Theme 2\u00a0<\/span><\/b>to understand what factors impact\u00a0<\/span>clinical progression and their respective trajectories of decline so that we can better appreciate where to\u00a0<\/span>invest and what is worth investing in individuals affected by VCI to reduce the economic burden and\u00a0<\/span>improve delivery of care to demented individuals. This work aims to a) generate estimates of the long-term\u00a0<\/span>health and economic impact of VCI by applying simulation modelling and external data sources; and b)\u00a0<\/span>investigate the economic impact including cost-effectiveness of VCI management in the context of the\u00a0<\/span>clinical trial of multidomain lifestyle interventions and use of diagnostic biomarkers .<\/span><\/p>\n <\/div>\n<\/div>\n\n
