He has a history of an abdominal surgery. He underwent EUS-FNA of the mass.<\/p>\n\t\t\t\t<img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-21.jpg\" alt=\"eus-fna 21\" itemprop=\"image\" height=\"1280\" width=\"1280\" title=\"eus-fna 21\" onerror=\"this.style.display='none'\"  \/>\n\t\t\t\t<img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-22.jpg\" alt=\"EUS-FNA case 5\" itemprop=\"image\" height=\"1280\" width=\"1280\" title=\"eus-fna 22\" onerror=\"this.style.display='none'\"  \/>\n\t\t\t\t<img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-23.jpg\" alt=\"eus-fna 23\" itemprop=\"image\" height=\"1280\" width=\"1280\" title=\"eus-fna 23\" onerror=\"this.style.display='none'\"  \/>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-0\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-0\">Cytologic findings<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-0\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t<p>&nbsp;<\/p>\n<ul>\n<li>Architecture: Sheets, small clusters (gland-like formation) and single cells<\/li>\n<li>Background: Dirty necrosis\u00a0<\/li>\n<li>Columnar cells with nuclear palisading in the larger sheets<\/li>\n<li>Increased N:C ratios<\/li>\n<li>Ovoid to elongated nuclei\u00a0<\/li>\n<li>Prominent nucleoli<\/li>\n<li>Coarse chromatin pattern<\/li>\n<li>Moderate nuclear pleomorphism<\/li>\n<\/ul>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-1\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-1\">Differential diagnoses<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-1\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t&nbsp;<br \/>\nThe presence of glandular formations, significant nuclear atypia, necrosis and atypical singly occurring cells point to a most likely diagnosis of adenocarcinoma. A pure pre-invasive primary lesion such as a neoplastic mucinous cyst (IPMN and MCN) is less likely, due to the presence of necrosis and single malignant cells; as well as the absence of background mucin.\u00a0\n<p>The main differential diagnosis would thus be primary ductal vs metastatic adenocarcinoma.\u00a0<\/p>\n<p>Other primary tumours of the pancreas including acinar cell carcinoma and solid pseudopapillary neoplasms are unlikely due to the presence of overtly malignant features and\u00a0 significant nuclear atypia.\u00a0<\/p>\n<p>The clinical history would thus be important in the diagnostic workup.<\/p>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-2\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-2\">Histologic and immunohistochemistry<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-2\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t&nbsp;<br \/>\nThe cell block shows glandular and sometimes cribriform structures lined by malignant glandular epithelium composed of tall columnar cells with elongated vesicular nuclei and prominent nucleoli. There is associated luminal necrosis.\n<table>\n<tbody>\n<tr>\n<td><img decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-24.jpg\" alt=\"\" \/><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<p>&nbsp;<\/p>\n<p>The tumour cells are CK7- CK20+ CDX2+ SATB2+, highly suggestive of colorectal type adenocarcinoma.<\/p>\n<table>\n<tbody>\n<tr>\n<td><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-25.jpg\" alt=\"\" width=\"150\" height=\"150\" \/><\/td>\n<td><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-26-150x150.jpg\" alt=\"\" width=\"150\" height=\"150\" \/><\/td>\n<td><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-27.jpg\" alt=\"\" width=\"150\" height=\"150\" \/><\/td>\n<td><img loading=\"lazy\" decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/eus-fna-28.jpg\" alt=\"\" width=\"150\" height=\"150\" \/><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-3\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-3\">Diagnosis<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-3\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t&nbsp;<br \/>\n<strong>Metastatic adenocarcinoma of likely colorectal origin<\/strong>\n<p>Based on the cytologic features alone, the clues suggestive of enteric differentiation include:<\/p>\n<ol>\n<li>Peripheral palisading of nuclei\u00a0\u00a0<\/li>\n<li>Tall columnar cellular morphology with elongated nuclei\u00a0<\/li>\n<li>Presence of dirty necrosis<\/li>\n<\/ol>\n<p>Bear in mind that adenocarcinoma from various visceral sites can exhibit enteric differentiation and the following can assist in suggesting a colorectal origin:<\/p>\n<ol>\n<li>Clinical correlation\n<ul>\n<li>The clinical history of previous colorectal adenocarcinoma and the presence of multiple metastatic lesions can be helpful.
