Hence, additional sampling for a cell block should be obtained from the FNA procedure, whenever a clinically aggressive salivary gland lesion is suspected. If further diagnostic characterisation is not possible, the tumour may be signed out as a &#8220;high grade carcinoma&#8221;. Importantly, even if the carcinoma cannot be specifically typed, the high grade nature of the tumour should be conveyed to the clinician so that radical surgery can be planned accordingly.<\/p>\n<p>In the salivary gland, a high grade malignant tumour may arise de novo or due to high grade transformation of a low grade malignancy e.g. adenoid cystic carcinoma or a benign tumour like pleomorphic adenoma. A distinct lower grade \/ benign component may be present on the smear.<\/p>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-1\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-1\">Cell Block and Histology<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-1\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t&nbsp;<br \/>\n<img decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/Sal10.jpeg\" alt=\"\" \/>\n<ul>\n<li>Degenerate clusters of malignant cells with hyperchromatic nuclei<\/li>\n<\/ul>\n<table>\n<tbody>\n<tr>\n<td><img decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/Sal11.jpeg\" alt=\"\" \/><\/td>\n<td><img decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/Sal12.jpeg\" alt=\"\" \/><\/td>\n<td><img decoding=\"async\" src=\"https:\/\/medicine.nus.edu.sg\/pathweb\/wp-content\/uploads\/2025\/06\/Sal13.jpeg\" alt=\"\" \/><\/td>\n<\/tr>\n<\/tbody>\n<\/table>\n<ul>\n<li>Invasive islands of malignant cells with central necrosis<\/li>\n<li>Moderately pleomorphic vesicular nuclei with prominent nucleoli<\/li>\n<li>Moderate to abundant eosinophilic cytoplasm<\/li>\n<\/ul>\n\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--label-2\" tabindex=\"0\" aria-controls=\"fl-accordion--panel-2\">Diagnosis and Take Home Points<\/a>\n\t\t\t\t\t\t\t\t\t\t\t<a href=\"#\" id=\"fl-accordion--icon-2\" tabindex=\"-1\"><i title=\"Expand\">Expand<\/i><\/a>\n\t\t\t\t\t&nbsp;<br \/>\n<strong>Salivary duct carcinoma<\/strong>\n<p>Take home points<\/p>\n<ul>\n<li>Marked nuclear atypia and severe nuclear pleomorphism are important features to categorise the tumour as high grade.
