A 56 year old man complaining of right upper quadrant abdominal pain radiating to the right scapula, fever and vomiting. A palpable, tender gallbladder was noted on physical examination.
The gallbladder is enlarged and the wall shows some thickening and oedema. A large rounded, brownish-black stone is seen in the neck of the gallbladder.
