Medication errors are among the top 3 most common causes of adverse events in healthcare worldwide. Errors can occur at any stage of managing a medication, from prescription to dispensation to administration. The use of an electronic medication system has reduced errors due to poor handwriting, non-standard dosing and adverse drug reactions. However, when humans take short-cuts, errors still arise, and electronic systems can give rise to their own special errors as well.

This session will highlight common/recurring medication errors with the objective of learning from these errors. Learning from medication errors that occur in our hospital and in other organistions is one of the best ways to prevent repeat occurences. In a culture of safety, all errors should be viewed as learning opportunities, and information gathered from error analyses should be used to improve medication safety processes.
