Paediatric heart failure is common in cardiac intensive care unit. Effective assessment and management needs to take into account not only for initial stabilisation, but longer-term outcomes and goals as well. This requires the practitioners to understand the underlying pathophysiology and the implications of acute treatment on prospects for myocardial recovery or eventual transplantation. The traditional approach of long-term inotropic support is potentially harmful for some patients and should be replaced with implementation of more standard heart failure treatment strategies. This talk will review these key principles and suggest specific approaches to these complex patients.
