A complete application would have fulfilled the following:<\/p>\n<ol style=\"list-style-type: lower-alpha;\">\n<li>all required fields are <span style=\"color: #000000;\">accurately <\/span>filled and all supporting documents are uploaded into the system<\/li>\n<li>a unique 10-digit application number is auto-generated by the system (we recommend that you jot down the number for reference)<\/li>\n<li>the non-refundable SGD76.30 payment is successful and supported by Tax Invoice (also known as the receipt) which is downloadable from the system<\/li>\n<li>an auto-generated confirmation of receipt of application from Graduate Admission System (GDA2) (auto-email will be received upon clicking of \"Submit\" button after payment)<\/li>\n<\/ol>\n<p>A completed application does not indicate a successful placement or admission into the programme.\u00a0 All applications and fees must be submitted by the closing date as stipulated above.\u00a0 Late or incomplete applications will not be processed. Hardcopy and email applications will be not accepted.<\/p>\n<p>The following supporting documents must be submitted in GDA2:<\/p>\n<ul>\n<li>Recent passport-sized photograph (image has to be 400 by 514 pixels)<\/li>\n<li>Letter of Enrolment\/Confirmation into Ophthalmology Residency Programme<\/li>\n<li>Certified true copy \/ scanned coloured copy of the following documents (Official English translations will be required for stamps or certificates sent that are not in English):\n<ul>\n<li>Identification documents:\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li><strong>NRIC<\/strong> : for locals \/ Permanent Residents<\/li>\n<li><strong>Employment Pass and Passport<\/strong> : for foreigners (residing in Singapore)<\/li>\n<li><strong>Passport <\/strong>: for foreigners (not residing in Singapore)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<\/li>\n<li>Recognised basic medical degree certificate (MBBS)<\/li>\n<li>Pass Letters of FRCOphth Part 1 Written, Refraction Certificate and\/or Part 2 Written (if applicable)<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p>Photocopies of certificates and\/or official translations (in English)* including any other official supporting documents will be accepted only if they have been prepared and\/or authenticated by one of the following:<\/p>\n<ul>\n<li style=\"list-style-type: none;\">\n<ul>\n<li>the issuing University or Medical School<\/li>\n<li>Medical Council of which you are registered and licensed to practice<\/li>\n<li>Government Ministries of Health<\/li>\n<li>a notary public or Justice of the Peace<\/li>\n<li>Division of Graduate Medical Studies (DGMS) - For authentication by DGMS office, original certificates and documents must be produced<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n<p><em>* Applicants must submit an authenticated translation if their official supporting documents are not in English. \u00a0A fresh ink stamp must be used to authenticate the copy or translation of the primary medical qualification.<\/em><\/p>\n<p>Applicants who fail to present their original documents for verification without acceptable reasons will not be admitted to the examination.\u00a0 Applicants whose original documents have been damaged or misplaced must apply to the relevant organisations \/ authorities for replacements or letters of verification.<\/p>\n<p><span class=\"ui-provider bsi bsj c d e f g h i j k l m n o p q r s t bsk bsl w x y z ab ac ae af ag ah ai aj ak\" dir=\"ltr\">An applicant is only allowed to enrol concurrently in another programme offered by NUS or another University only if granted approval from the Vice Dean of Faculty and the Head of Department. The Degree Programme refers to any programme (including examination-only programmes) which leads to NUS Degree conferment. DGMS is not obliged to provide reasons for unsuccessful applications. Other respective rules\/regulations of DGMS apply.<\/span><\/p>\n<h5><\/h5>\n<h5><span style=\"color: #ef7c00;\"><strong>Withdrawal Policy<\/strong><\/span><\/h5>\n<p>Candidates must contact the respective Officer-in-Charge (OIC) as soon as they know that they are withdrawing from the programme.\u00a0 Requests to withdraw from the programme must be made in writing.\u00a0 Emails are acceptable.<\/p>\n<p>Candidates who notify the Division that they would like to withdraw from the programme and seek a refund must provide the supporting evidence where appropriate.\u00a0 The Division will generally consider providing refunds to candidates under the following circumstances:<\/p>\n<ul>\n<li>Illness - Refund request must be accompanied by a medical certificate.<\/li>\n<li>Pregnancy - Refund request must be accompanied by medical report.<\/li>\n<li>Accident e.g. victim of crime, accident or disaster -
