If you are not issued with a staff card or coming into the department as a visitor, a temporary card can be applied online.<\/td>\n<\/tr>\n
The NUS Medicine Card Access Application System has been launched and can be accessed via this link<\/a> and user guide<\/a>.<\/td>\n<\/tr>\n
Request Safety lead or PI to apply on your behalf after completion of (1) Safety Induction Online Quiz (passed with >80%) and (2) fill in and complete the\u00a0Safety and Health Orientation Form<\/a>.<\/td>\n<\/tr>\n

All personnel exiting the department, are to complete the exit declaration form and submit it to Bchsafety (mailbox 22 next to reception counter) before returning your Staff Card \/ Temp Card \/ i-MediCare Card \/ NUS 16GB secured flash drive to General Office.<\/td>\n<\/tr>\n
