1) Emergencies
2) Accidents/incidents/near-misses/unsafe practices
3) Suggestions and feedback

Please notify the Departmental Safety Committee immediately at the above phone numbers/e-mail to report any emergencies, accidents, incidents, near-misses or unsafe practices.

Please also notify OSHE via AIMS (Accident/Incident Management System) within 24 hours to report any accidents, incidents or near-misses. Please update your lab’s Accident/Incident Register thereafter and file it in your lab’s Safety Management System.
