Speakers: Dr Loi Hoi Yin, Patient Safety Officer, Senior Consultant, DDI, NUH & Dr Chew Kean Lee, Patient Safety Officer, Consultant, Lab Medicine, NUH

Laboratory and radiology investigations play an important part in diagnosing and treatment of patients. DDI sees 13,000 inpatints and outpatients per month on average and strives to provide the best diagnostic quality images and accurate reports to answer specific clinical questions. The clinical information conveyed to the radiologists ideally has to be concise, current and relevant and by these means, able to convey an indication for a particular type of scan.

In a similar fashion, the lab receives thousands of investigation orders. Pre-analytical errors hinder the lab from providing results accurately and in a timely fashion. Providing accurate information will ensure not only safety for the patient but indirectly will translate to prompt and effective management of the patient.
