Deadline for online application and submission of the printed application package to Division of Graduate Medical Studies must be done by 2nd May 2018.
The intake assessment will be held tentatively on 26 May 2018 or 2 June 2018.

Applicants should complete all the sections (i.e. personal particulars, contact, and academic qualifications, all the way through the declaration page).
After completing the online submission, applicants must print their submitted application, sign and forward to:

Attach one passport-size photograph together with the goal statement, copies of transcripts & degree certificates, 2 referee reports in sealed envelopes (where applicable), TOEFL/IELTS certificates (if any), and application fee payment receipt.
Please make payment via NUS Graduate Admission System (GDA2). Click on payment tab to access payment gateway. Include the payment receipt with the application package when submitting to NUS. Do note that your application will only be processed upon receipt of the application fee.
Copies of transcripts of academic work (grades / marks) starting from Secondary School Education (GCE “O” level, GCE “A” Level or equivalent) and degrees / diplomas / certificates from each college or university previously attended must be submitted. If credentials are not in English, they must be accompanied by a notarized translation.
Original documents should not be submitted as they will not be returned. However, original educational certificates/transcripts/degree scrolls must be furnished for verification when applicant is shortlisted for interview.
Applicants currently in their final semester can produce their available transcripts with the application package and must produce the final semester transcript as soon as they have obtained them.
Applicants whose undergraduate degree was not in English are required to submit TOEFL /IELTS scores. Applicants should have a score of at least 620 for the TOEFL paper-based test or 7.5 for IELTS.
Goal Statement form can be downloaded here. Completed forms are to be uploaded via document upload tab in GDA2 system and hard copy of the form is to be attached with the printed application form.
TWO Referee names, preferable one academic and another employment are to be assigned in the GDA2 system which will trigger email to the respective referees to complete online. Monitor the system to ensure Referee completes the report online within the system generated deadline. Hard copy submission is not required if referee submits online.
Should referee encounter problem completing/submitting online, applicant can forward soft copy of the referee report form to him/her. In this instance, request referee to submit completed form via email to Ms. Mani directly at firstname.lastname@example.org.
Ensure referee names are not omitted on the printed online application. Should referee wish to post completed report to you directly, do request to place the document in sealed envelope, signed on the envelope cover flap and include this with your application package.
